This form needs to be completed for the nomination of two suitably qualified External Examiners (EE) by the Office of the Dean, School of Graduate Studies, and should be forwarded to this office once the Chair of the Supervisory Committee has made contact with the EE to ascertain their willingness to serve as an EE and be able to complete the duties within 6 weeks of receipt of the thesis. Supervisory Committee Chairs should consult the Conflict of Interest Policy to guide in the selection of EE prior to making contact with potential examiners.
