Health Requirements for Clinical Rotations

Students must have a confirmed placement letter to begin clinical training. Before the Office of Clinical Education Operations can issue a confirmed placement letter, all mandatory health requirements must be completed, documented, and cleared. 

Students must upload their Health Form and any applicable documents through the Student Health Records Portal (SHRP) using the “Upload File” function. All health records, including any corresponding laboratory results, must be combined into a single PDF document. Submissions with multiple PDF files may be rejected. Students should retain original copies of all health records, as they will be required for future residency applications and credentialing processes.

These requirements are mandatory for all healthcare workers and are intended to comply with public health and hospital regulations designed to protect patients, healthcare personnel, and the clinical learning environment. 

SGU health requirements have three parts:

Part I: HEALTH HISTORY

Students are required to complete and sign a current personal history form within six months prior to the start of clinical rotations.

Part II: PHYSICAL EXAM

Students must have a physical examination completed within six months prior to the start of their first clinical rotation. The SGUSOM physical exam form must be completed, dated, and signed by the student's personal physician, nurse practitioner or physician assistant.
